The referral desk

Send a referral through the secure link

The link opens the practice's own secure form. Patient details, scans and notes go in there, and none of it is typed into this website. A person at the desk then calls the patient to book.

What the form asks for

What goes in the referral, and where

  • 01

    Who the patient is

Name and phone number, and your office's name and contact, so the desk can call the patient and write back to you.

  • 02

What you'd like us to see

Your reason for the referral, in your own words, entered in the secure form.

  • 03

    Scans and records

Uploaded in the secure form. This website never takes images, records or health history.

A desk that works for your office

One phone number, one link, one person who calls the patient.

The desk books the first visit, tells the patient what to bring and keeps your office's contact for the note that follows. If you would rather talk before you refer, call and ask for a surgeon.

What your office can expect

  • The surgeon explains options and risks at the first visit

  • A written note goes to your office after the visit

  • The patient returns to you for restorative care

Before you refer

Can you see a patient quickly?

Call the desk and tell us it is urgent. We make room when we can, and tell you plainly when we can't. For a serious injury, call 911 or go to an emergency room.

Do you take the patient's insurance?

The desk checks the patient's dental and medical benefits before the first visit. How fees work is on its own page.

Do you take referrals from physicians?

Yes. Physicians refer patients for biopsies, jaw problems and teeth that need to come out before other treatment.

Who restores an implant?

The patient's dentist or a restorative dentist you name. The surgeon here places the implant.

Do you send the images back?

We send a written note. If you need the scans, ask the desk.

Care here is provided by oral and maxillofacial surgeons

Each surgeon's training and license are listed on the About page.
