For patients
Your first visit, step by step
- 01
Step 1: The desk calls you
If your dentist or doctor referred you, the desk calls to book. You can also call us yourself.
- 02
Step 2: You fill in forms before you come
Your health history goes in the patient portal, not on this website. Bring your insurance cards and a list of your medicines.
- 03
Step 3: The surgeon reviews your scan
At the visit the surgeon looks at your scan, asks about your health and explains the options and the risks.
- 04
Step 4: You get a written estimate
Take it home. You decide when you are ready, and the desk can answer questions by phone.
Fill in your forms in the patient portal
The portal is the practice's own. Your health history stays there, not on this website. If you'd rather talk to a person, call the desk.

Before your first visit
Do I need a referral?
Many patients come from a dentist or physician, and the desk can take the referral for you. If you don't have one, call and ask.
What do I bring to the first visit?
Your insurance cards, a list of your medicines and the scan if your dentist gave you a copy.
Will I have surgery at the first visit?
Usually the first visit is a consultation. The surgeon tells you when surgery can be scheduled.
Can someone come with me?
Yes. Bring a family member or a friend if you like.
Send a patient, or ask for a visit
Dentists and physicians use the secure referral link. Patients can write to the desk or call.
